I affirm that I am the legal guardian of the minor named above and have legal authority to consent to a massage on their behalf. I further attest that I have discussed this massage with this minor and given them the opportunity to ask questions and decide for themself if they consent to receiving massage. I agree that Misty Murray, LMT, will discuss the goals and limitations of professional massage with the minor named above prior to the service, and will answer any further questions during verbal intake. I understand that I must remain on the premises for the duration of the massage service. For minors under 14, the guardian must be present in the massage room. For minors 14 and above, the guardian may remain in the waiting room or in the massage room, at their discretion. I affirm that I have given an accurate description of any concerns for the minor named above and that they do not have any ailments or conditions that would render massage incompatible with their health or well-being, or that of the massage therapist. I release Misty Murray, LMT, of any liability for any injury sustained on the premises. I understand that massage is not a replacement for medical care.
